How to be setup on the Homecare system

Print off and complete the consent form which gives permission for your details to be sent to Homcare.

If you would like Homecare to pick up your monthly prescription from your GP please tick the box on the form.

Return this form to your dietitian.

Your dietitian will complete a registration form and send to Homecare.

You will then be registered on the Homecare system and sent a welcome information pack exlpaining the service in more detail.

Your dietitian will contact your GP to request your initial prescription.
Your Homecare coordinator will call you to arrange delivery of your Vitaflo products which will be sent direct to your chosen address each month.
You will then receive your first delivery on your requested day. 
Your Homecare coordinator will follow up with a courtesy call to check you are happy with everything and to answer any questions you may have.

This will happen each month.
