Your step by step guide to Homecare

	Post introduction pack to the patient: Explanation letter. Introduction leaflet explaining the new service. Consent form. Addressed envelope for patients to return forms to you.
	You receive a consent form back from the patient, then complete either patient registration form (pku) or patient registration form (other metabolic conditions) and send to Homecare.
	Patient is registered on Homecare and forwarded a welcome pack.
	Contact the patient's GP to request the initial prescription - standard letters provided.
	Homecare send GP letter requesting prescription or patient collects own prescription and sends to Homecare.
	Homecare coordinator rings patient and arranges delivery.
	Homecare inform you of patient's first delivery date - this is confirmation that patient is set up on Homecare.
	Patient receives first delivery.
	Homecare sends monthly report to you regarding your patients orders.
